改良Goldberg手术治疗成人直肠完全脱垂临床观察

目的 评价改良Goldberg手术（直肠缝合固定加乙状结肠部分切除吻合术）治疗直肠完全脱垂的有效性,为进行临床推广提供临床循证医学证据.方法 回顾性分析2011年3月~2013年6月间在成都中医药大学附属医行经改良Goldberg手术治疗完全直肠脱垂的患者的13例,观察患者术后恢复及随访症状改善情况,从而评价该手术方式的有效性.结果 13例患者手术均获得成功,术后除1例大便失禁末明显改善,余下患者术前直肠脱垂的便秘、排便困难、大便失禁、便血等症状相关症状全部消失.术后随访3~12个月未见复发病例.结论 改良Goldberg手术方式重建了盆底的解剖结构,不仅解决了直肠乙状结肠本身的病变,而且改善了盆底的功能,降低了术后复发率,取得满意疗效.

Clinical study of modified Goldberg to rectal prolapse patients

Objective To study the effect of modified Goldberg operation to rectal prolapse. It also provides support of EBM for further clinical practice. Methods This study involved 13 rectal prolapsed cases which were treated in our hospital from March 2011 to June 2013 with modified coldberg. Retrospective study were used for evaluation by postoperative improvements observation and follow-up visits. Results 12 cases had symptoms disappeared including constipation, difficult passing stool, fecal incontinence, hematochezia. There were no after 3 - 12 months. Only 1 case remained incontinence of feces after operation. Conclusion Modified Goldberg operation improves pelvic floor function and reduce postoperative reoccurrence by restoring the pelvic floor antomy.
