痔上粘膜点状结扎治疗重度痔病

目的探讨痔上粘膜点状结扎术治疗重度痔病的临床应用价值。方法选择同期重度痔病患者,分成两组,分别进行痔上粘膜点状结扎术(179例)和PPH技术(18例),比较两种术式的门诊/住院比、术中出血、术后疼痛、出血、恢复劳动时间及近期疗效等。结果两组均未出现术后出血、排便异常和肛门狭窄。痔上粘膜点状结扎术组治愈率较PPH术组稍低,但所需住院率低,手术时间短,术中出血少,术后疼痛轻,恢复劳动快,且费用低廉。结论痔上粘膜点状结扎术治疗痔病疗效肯定,可作为首选方法,对不能治愈的患者再选PPH或传统手术。

Punctuated Ligation of Mucous Membrane of Upper Haemorrhoid for Treatment of Severe Haemorrhoids

Objective To investigate the clinical application value of punctuated ligation of mucous membrane of upper haemorrhoid for treatment of severe haemorrhoids.Methods 197 patients with severe haemorrhoids were randomly divided into the punctuated ligation of mucous membrane of upper haemorrhoid group(179 cases) and procedure for prolapsed haemorrhoids(PPH) group(18 cases) from December 2001 to March 2005 in our hospital.The ratio of outpatient patients,hemorrhage in operation,algetic sense and hemorrhage after operation,recovery time for work and near curative effect were compared.Results All patients in both groups had no hemorrhage,abnormal defecate and anal straitness after operation.Although the healing rate in punctuated ligation group was lower than that of PPH group,operation time was shorter,less hemorrhage in operation,milder algetic after operation,sooner recovery for work,and much less fee.Conclusion The effect of punctuate ligation of mucous membrane of upper haemorrhoid for treatment of haemorrhoids is affirmed and so the procedure can be the first choice for patients with severe haemorrhoids.PPH or traditional operation could be chosen secondly.
