蛛网膜下腔出血后脑血管痉挛与内皮一氧化氮合酶基因的多态相关研究

摘    要:目的 探讨蛛网膜下腔出血后发生脑血管痉挛的风险是否与内皮一氧化氮合酶基因的多态有关.方法 选择3组蛛网膜下腔出血患者:动脉瘤性蛛网膜下腔出血发生脑血管痉挛组(n=98)、外伤性蛛网膜下腔出血发生脑血管痉挛组(n=96)和动脉瘤性或者外伤性蛛网膜下腔出血但未发生腩血管痉挛组(n=195).病例组194例和对照组195例中的100例的亲代也被检查且采用传递不平衡分析,一种基于家族的研究设计以便检验相关性.结果 检查了内皮一氧化氮合酶基因的4种多态,在病例对照比较中有两种多态与蛛网膜下腔出血后发生脑血管痉挛有关:在启动子的-786位点上一个T变成C的替换和在内含子4内的a缺失或b的插入.对于前者,C等位基因纯合体比另外两种基因型诱发脑血管痉挛的风险更高(优势比2.8,95%CI:1.4 to 5.6).对于后者的多态,携带a缺失的个体比不携带的个体患病风险更高(优势比2.3,95%CI:1.3 to 4.0).两种多态作为单倍型一起以家系为基础的研究采用传递不平衡分析进行分析.C或者a缺失单倍型从杂合体亲代传递到成为有蛛网膜下腔出血且脑血管痉挛比期望的频率更高(P=0.005).结论 内皮一氧化氮合酶的DNA序列的差异与蛛网膜下腔出血后脑血管痉挛的发生有相关性.
