激光气化治疗腰椎间盘突出症的护理

目的探讨经皮激光椎间盘减压术治疗腰椎间盘突出症的护理方法及近期疗效。方法腰椎间盘突出症患者80例，按随机数字法分成两组：常规护理组患者40例，采用常规护理方法，做好患者的围手术期护理工作。加强护理组患者40例，常规护理的基础上，系统的针对每位患者的状况进行心理干预、健康教育、护理指导和康复训练指导。治疗后1周、6和12个月对两组患者疗效进行随访。视觉模拟评分（visual analogue scale，VAS）值疗效评定法和日本矫形外科协会疗效评定（Japanese Orthopaedic Association，JOA）值进行疗效判定。结果两组患者治疗后1周，6和12个月的VAS值均较治疗前明显降低（P〈0．01），JOA值均较治疗前明显提高（P〈0．01）。加强护理组治疗后12个月VAS值和JOA值较常规护理组明显改善（P〈0．05）。治疗后12个月的优良率分别为常规护理组82．5％，加强护理组92．5％，加强护理组治疗后12个月的疗效优于常规护理组。结论经皮激光椎间盘减压术是治疗腰椎间盘突出症有效的方法之一，围手术期的护理及术中配合是保证治疗成功的重要组成部分。

Nursing Care for Patients with Lumbar Disc Herniation Undergoing Percutaneous Laser Disc Decompression

Objective To explore the effect of percutaneous laser disc decompression （PLDD） on lumber disc herniation and the nursing methods concerned. Methods Eighty patients with lumbar disc herniation were randomly divided into the conventional care group （40 cases） and intensive care group （40 cases）. The effect was assessed 1 week, 6 months and 12 months after the treatment respectively. Results For both groups, the VAS values at week 1, month 6 and month 12 after the treatment were remarkably lower than those before the treatment （ P 〈 0.01 ）. Meanwhile, the JOA values in both groups were improved significantly after the treatment （ P 〈 0. 01 ）.Compared with the group of conventional care, the group of intensive care showed greater improvement of VAS and JOA scores after the treatment （P 〈 0. 05 ）. The excellent and good rates of therapy at Month 12 Were 82. 5 % in the group of conventional care, and 92.5% in the group of intensive care. It was proved that the therapeutic effect of the group of intensive care was better than the group of conventional care. Conclusions PLDD is one of the effective treatments for lumber disc herniation, and the perioperative care and nursing during PLDD is essential to the success of the treatment as well.
