摘    要:目的 分析ICU病房重症感染患者致病微生物临床特点 ,探讨其致病微生物的新特点及其治疗途径。方法 观察 176例院内感染的分布情况、致病微生物的种类、耐药性、真菌及院内感染的发生率。结果 铜绿假单胞菌和肺炎克雷伯菌为ICU病房的主要致病微生物 ,在ICU病房内耐药菌明显增加 ,β -内酰胺酶 (ESBL)阳性菌较普通病房增多 ,真菌感染率和院内感染率有明显上升。结论 合理使用抗生素 ,避免使用能诱导或加重细菌产生耐药性的药物 ;早期经验性抗真菌治疗以降低真菌感染发生率 ;进一步加强ICU病房的管理以降低院内感染的发生率。

Abstract:Objective To analyze the clinical characteristics of bacteria isolated from 176 cases in ICU, and to discuss the new treatment for the patients in ICU. Methods The distributing of infection, kinds of bacteria, drug resistance, and the incidences of fungus infection and nosocomial infection were observed Results Pseudomonas aeruginosa and Klebsiella were the main bacteria in our ICU. The drug resistant bacteria, the incidences of fungal infection and nosocomial infection are increasing. There are more ESBL positive bacteria in ICU than in general ward. Conclusions In order to avoid the increase of drug resistant bacteria, the antibiotic should be used reasonablely. Anti-fungus drugs should be used early by experience. In order to reduce the incidence of nosocomial infection, administration of ICU should also be strengthened.
