原发性脾脏肿瘤(附16例分析)

目的：探讨原发性脾脏肿瘤的诊断与治疗。方法：回顾分析仁济医院１９９０年７月至１９９８年７月间原发性脾脏肿瘤１６例，其中良性８例，恶性８例，全部行脾脏切除，３例附加胰腺体尾切除。结果：全组无手术死亡，脾良性肿瘤预后良好，随访５例恶性淋巴瘤病例，３例分别于术后７个月，９个月和１８个月死亡，死于肿瘤复发，１例生存８年健在，１例术后５个月仍在术中，结论：原发性脾脏肿瘤早期诊断很 重要，治疗选择及预后与病理

Primary splenic neoplasms:an analysis of 16 cases

Objective:To investigate the diagnosis and treatm ent for primary splenic neoplasms.Meth- ods:Sixteen patients with primary splenic neoplasm s were retrospectively analyzed between July1990 and July1998,in which8cases were benign and8were m alignant.Splenectom y was performed in all cases,and distal pancreatectomy perform ed in3cases.Results:There was no operative death.The prognosis of benign splenic neoplasms was good.Five of8cases with primary lym phom as were followed up.Three of5 cases died from recurrence of neoplasms during postoperative7m onths,9months and18months respectively and one of5 cases has been alive for8years,one of5 cases is undergoing chem otherapy postoperative5 m onths. Conclusion:Early diagnosis for primary splenic neoplasms is very important,the option of treatment and progress are associated with pathologic types and stages for primary splenic neoplasms.
