74例Ⅱ～Ⅲ期喉鳞癌术后放疗与单纯手术疗效的比较与分析

目的探索Ⅱ～Ⅲ期喉鳞癌更合理的治疗方法，并评价其疗效。方法74例Ⅱ～Ⅲ期喉鳞癌按治疗方法分为单纯手术组42例和手术＋放射治疗组32例，采用SPSS10．0寿命表法计算生存率，各组间生存率的比较采用χ^2检验。结果74例Ⅱ～Ⅲ期喉鳞癌患者3年累积生存率74．32％，5年累积生存率68．92％。其中声门型喉鳞癌的5年累积生存率72．50％，声门上型喉鳞癌的5年累积生存率43．75％；单纯手术组的5年生存率71．43％，手术＋放疗组的5年生存率为65．63％。比较喉鳞癌单纯手术组和手术＋放疗组的5年累积生存率，Ⅱ～Ⅲ期均无统计学意义（P〉0．05）。结论Ⅱ～Ⅲ期喉鳞癌治疗以手术治疗为主，手术＋放射治疗组5年累积生存率与单纯手术治疗组疗效相当。

Objective To study the efficacy of different treatment methods for stage Ⅱ～Ⅲ laryngeal squamous cell carcinoma （LSCC）. Methods 74 stage Ⅱ～Ⅲ LSCC patients were retrospectively analyzed. Of the 74 patients, 42 were treated with surgery alone and 32 were treated with surgery and postoperative radiotherapy. The overall survival rate were observed. Results The 3 and 5-year overall survival rates were 74. 32% and 68.92%, respectively. The 5-year survival rates were 72.50% in glottie carcinoma and 43.75% in supraglottic carcinoma and were 71.43% in surgery alone and 65.63 % in surgery and radiotherapy group. There was no significant difference between surgery alone group and surgery plus radiotherapy group （P〉0. 05）. Conclusion The overall survival rates of surgery and radiotherapy and surgery alone are not different.
