小切口甲状腺切除术583例分析

摘    要:目的探讨小切口甲状腺切除术的可行性及优点。方法回顾性分析采用小切口甲状腺切除术治疗的583例甲状腺结节患者的临床资料及手术效果。结果手术均获成功。手术切口长度3-5cm,手术时间40-100min,平均60min。术中出血20-150ml,平均50ml。双侧甲状腺全切除术45例,一侧腺叶、峡部、对侧腺叶次全切除术118例,一侧腺叶、峡部切除术75例,一侧腺叶切除术127例,双侧腺叶次全切除术106例,一侧腺叶次全切除术112例。4例术后出现声嘶,2例术后出现饮水呛咳,2例术后出现抽搐。结论小切口甲状腺切除术安全可行,创伤小,可以在基层医院推广应用。

Abstract:Objective To investigate the feasibility and advantage of mini - incision thyroidectomy. Methods Clinical data and surgery effect of mini - incision thyroidectomy in 583 patients with various types of thyroid nodules were analysed retrospectively. Results All mini - incision thyroidectomies were successful. The length of skin incisions, operation time, blood loss were respectively 3 -5 cm, 40 - 100 min, 20 - 150 ml. Total resection of bilateral thyroids was performed on 45 cases, subtotal resection of lateral lobe, isthmus and bilateral lobes on 118 cases, resection of lateral lobe and isthmus on 75 cases, resection of lateral lobe on 127 cases, subtotal resection of Bilateral lobes on 106 cases, subtotal resection of lateral lobes on 112 cases. There was appearance of hoarseness in 4 cases, drinking suffocation in 2 cases and convultion in 2 cases after thyroidectomy. Conclusion Mini -incision thyroidectomy is technically feasible and safe enough to be popularized to primary hospitals.
