慢性阻塞性肺疾病急性加重、肺心病的临床观察

目的探讨低分子肝素钙联合川穹嗪、糖皮质激素治疗慢性阻塞性肺疾病(COPD)的可行性及安全性。方法把2008～2011年收治的200例慢性阻塞性肺疾病患者随机分为两组。对照组:采用低流量持续吸氧、抗感染、化痰及体位引流排痰、扩张支气管等对症处理。观察组:在对照组治疗的基础上加用低分子肝素钙、川穹嗪、糖皮质激素治疗。结果一周后,观察组患者的咳痰喘症状,右心功能不全,生活质量好转率好于对照组;血气分析检查:观察组患者的动脉血氧分压、血氧饱和度改善率好于对照组;6min步行试验:观察组好于对照组;未出现黏膜出血等明显不良表现。结论采用低分子肝素钙联合川穹嗪、糖皮质激素治疗COPD是可行的和安全的。

Clinical Observation on Effect of Low Molecular Heparin Calcium,Ligustrazine and Glucocorticoid for Treatment of Chronic Obstructive Pulmonary Disease and Pulmonary Heart Disease

Objective Discussion on the feasibility and safety of low molecular heparin calcium and ligustrazine for treatment of Chronic obstructive pulmonary diseases.Methods 200 COPD patients received from 2008-2011were randomly divided into two groups.Control group（CG）： Rescue therapy of continuous low-flow oxygen inhalation,anti-infective therapy,reducing phlegm and postural drainage were given for this group.Observation group（OG）： Low molecular heparin calcium,ligustrazine and glucocorticoid were added to the therapy of this group beyond the treatment for CG.Results After treatment for one week,patients in OG are better than CG in the expectoration or asthma symptoms,right ventricular dysfunction and quality of life improvement.Blood gas analysis results show that improvement for arterial partial pressure of oxygen and oxyhemoglobin saturation is better for OG.It also shows that OG get better performance during six-minute walk test results.No advent of symptoms,such as mucosal bleeding,is observed.Conclusion It is possible and safe for the therapy of COPD using low molecular heparin calcium,ligustrazine and glucocorticoid simultaneously.
