摘    要:目的通过对行血管超声引导下结合微插管鞘技术(MST)的PICC在肿瘤患者中的应用效果进行观察及经验总结,探讨其优越性。方法选择80例患者,随机分为观察组(n=40)与对照组(n=40)。试验组患者采用血管超声引导系统评估血管,利用MST技术行上臂PICC穿刺;对照组患者在肉眼下穿刺血管行PICC。结果观察组置管一次成功率为97.5%,且一次成功率、穿刺点出血、静脉炎的发生率均低于对照组(P<0.015),导管移位对比差异无统计学意义(P=0.152)。结论血管超声引导下结合MST用于PICC具有切实可行的效果,值得在临床中广泛开展,尤其适用于血管条件不好的患者。

Abstract:Objective To sum up the experience of modified seldinger technique(MST) in the application of advanced cancer patients,and explore the superiority of MST.Methods 80 patients was divided into two groups,observational group use MST and vascular ultrasound guidance system,contrast group use traditional method.Results The first intubation success rate was 97.5%,and puncture bleeding、phlebitis were lower than contrast group(P〈0.015).There didn't have a dinstinct difference in catheter shift(P=0.152).Conclusion MST with ultrasound guided has a useful effect,it should be widely carried out in clinical,especially for those person whose vein isn't very well.
