前方肌肉间隙入路髋关节置换术的疗效观察

目的探讨前方肌肉间隙(OCM)入路髋关节置换术临床应用价值。方法选取2017年1月至2018年1月于本院行髋关节置换术的68例患者作为研究对象,根据手术方式不同将其分为观察组和对照组,每组34例。对照组行传统髋关节置换术治疗,观察组行OCM入路髋关节置换术治疗,比较2组患者围术期指标,包括手术时间、术中出血量、手术切口长度、术后下床时间、住院时间等;采用Harris评分和WOMAC评分量表评估患者关节功能改善情况。结果患者手术时间组间比较差异无统计学意义(P>0.05),观察组患者在术中出血量、手术切口长度、术后下床时间、住院时间方面明显优于对照组,差异有统计学意义(P<0.05)。术前2组患者Harris评分及WOMAC评分比较,差异无统计学意义(P>0.05);术后1周、12个月观察组Harris评分及WOMAC评分均明显高于对照组,差异有统计学意义(P<0.05)。结论OCM入路微创髋关节置换术具有创伤小、术后恢复快、预后质量高等优点,适用于多数髋关节疾病患者。

Therapeutic effect of anterior muscle space approach for hip arthroplasty

Objective To explore the clinical value of hip arthroplasty with anterior muscle space approach,namely the Orthop?edische Chirurgie Müenchen(OCM)approach.Methods Totally 68 patients who underwent hip arthroplasty in our hospital from January 2017 to January 2018 were selected as the research objects.They were divided into the observation group and the control group according to different surgical methods,with 34 cases in each group.The control group received conventional hip replacement surgery,and the observation group received OCM approach hip replacement surgery.The perioperative indicators of the two groups were compared,including the operation time,intraoperative blood loss,surgical incision length,postoperative time of getting out of bed,length of hospital stay.The Harris score and WOMAC scale were used to evaluate the improvement of joint function.Results There was no significant difference in operative time between the two groups(P>0.05);the intraoperative blood loss,surgical incision length,postoperative time of getting out of bed,length of hospital stay of the observation group were significantly better than those in the control group,and the differences were statistically significant(P<0.05).Before operation,there was no significant difference in Harris score and WOMAC scores between the two groups(P>0.05).The Harris scores and WOMAC scores of the observation group were significantly higher than those of the control group at 1 week and 12 months after the operation,with statistically significant differences(P<0.05).Conclusion OCM approach minimally invasive hip arthroplasty has the advantages of less trauma,rapid postoperative recovery,and high quality of prognosis,which is suitable for most patients with hip joint diseases.
