摘    要:目的观察基于解剖结构的改良置胃管法,即呼气配合旋转置胃管法在脑卒中气管切开患者中的应用效果.方法选择2019年3-12月某院收治的脑卒中气管切开拟行留置鼻胃管的患者67例,按患者入院时间的先后顺序分为对照组32例和试验组35例.对照组应用常规胃管置入法,试验组应用基于解剖结构的改良置胃管法.对比两组方法胃管置入成功率、首次置入平均耗时、置入不良反应的发生率.结果试验组患者胃管置入一次成功率和总成功率均高于对照组,胃管首次置入耗时、胃管置入时患者的不良反应发生率均低于对照组,差异均有统计学意义(均P<0.05).结论基于解剖结构的呼气配合旋转置胃管法对脑卒中气管切开患者的应用效果优于常规胃管置入法,值得临床推广应用.

Abstract:Objective To observe the effect of the improved gastric tube placement method based on anatomical structure,that is,exhaling combined with rotating gastric tube method in patients with tracheotomy after stroke.Methods From March to December 2019,67 patients who were admitted to a hospital for tracheotomy were divided into control group(32 cases)and experimental group(35 cases)according to the time sequence of admission.The control group was treated with conventional gastric tube placement method,while the experimental group was treated with modified gastric tube placement method based on anatomical structure.The success rate,average time and adverse reactions of gastric tube placement in two groups were compared.Results The success rate and total success rate of gastric tube implantation in the experimental group were higher than those in the control group;and the time of the first gastric tube implantation and the incidence of adverse reactions in patients with gastric tube implantation were lower than those in the control group(all P<0.05).Conclusions The effect of exhalation based on anatomical structure combined with rotating gastric tube placement method on patients with tracheotomy after stroke is better than that of conventional gastric tube placement method,which is worthy of clinical application.
