高频喷射通气对控制呼吸的实验研究

摘    要:采用不同部位和条件的高频喷射通气(HFJV)对25条正常或油酸型杂种犬在自主呼吸消失后进行控制呼吸。结果表明:气管内中条件(DP=120kPa,喷射气量=11L/min)和咽喉部高条件(DR=250kPa,喷射气量=25L/min)的HFJV,在通气4h内,均能使PaO2显著升高,维持在正常或正常以上水平,且元CO2潴留;同时,在不改变肺血管外水量(EVLW)的情况下,降低肺内分流(Qs/Qt),以气管内HFJV更为显著,提示HFJV能有效地对油酸型杂种犬进行控制呼吸。

Abstract:The controlled breathing was conducted with the high frequency jet ventilation (HFJV)jetting different loctions and with diffe-rent parameters on 25 apneic dogs, whose lungs were normal or injuried by oleic acid. The results showed: HFJV: jetting at intracheal with middle condition (driving pressure= 120 kPa, jet volume=11 L/min)and at pharyngolarynx with high condition (driving pressure= 250 kPa, jet volume=25 L/min)cousd make the PaO_2 increase significantly and the PaCO_2 remain in the normal range within the four hours experiment. Meanwhile, the HFJV, especially jetting at intracheal, could make the pulmonary shunt (Qs/Qt)decrease and did not reduce the extravacular lung water (EVLW). It is suggested that the HFJV can control the breathing effectively on the apneic dogs with injuried lungs by oleic acid.
