慢性心力衰竭患者和肽素及N端前体脑钠肽与左室射血分数的变化

目的 研究血浆和肽素(copeptin)、N端B型钠尿肽前体(NT-proBNP)水平、左室射血分数(Left Ventricular Ejection Fraction,LVEF)和慢性心力衰竭(Chronic Heart Failure,CHF)及其严重程度之间的关系.方法 将100例CHF患者按NYHA分级分组,NYHAⅡ级组35例,Ⅲ级组32例,Ⅳ级组33例,并设立30例健康对照组,测定Copeptin、NT-proBNP水平和LVEF,将各级心功能患者的Copeptin、NT-proBNP水平和LVEF分别与对照组进行比较,观察Copeptin、NT-proBNP水平和LVEF的关系.结果 CHF患者血浆Copeptin、NT-proBNP水平与对照组相比显著升高(P<0.01),并随NYHA分级的增加而升高.Copeptin与NT-proBNP呈显著正相关(r=0.641,P<0.01),Copeptin与LVEF呈负相关(r=-0.497,P<0.01),NT-proBNP与LVEF呈负相关(r= -0.518,P<0.01).结论 血浆Copeptin、NT-proBNP对CHF患者的诊断、病情判断、愈后评估具有一定临床意义.

Changes of copeptin,NT-proBNP and left ventricular ejection fraction in patients with chronic heart failure

Objective To research the relationships between the serum copeptin,NT-proBNP and Left Ventricular Ejection Fraction（LVEF）in patients with chronic heart failure,to observe the associativity between the levels of the serum copeptin,NT-proBNP and LVEF.Methods 100 patients with CHF were grouped into NYHA classification,NYHA II-level group of 35 cases,third-level group of 32 cases,IV-level group of 33 cases,and 30 healthy control group,level of determination of Copeptin,NT-proBNP,and LVEF,all levels of cardiac function in patients with Copeptin,NT-proBNP levels and LVEF compared with the control group,respectively,Copeptin,NT-proBNP levels and LVEF of observation.Results Copeptin,NT-proBNP levels in plasma in patients with CHF increased significantly compared with the control group （P〈0.01）,and rise with the increasing NYHA classification.Copeptin and NT-proBNP were notable （r=0.641,P〈0.01）,Copeptin and LVEF negative correlation （r=-0.497,P〈0.01）,NT-proBNP and LVEF negative correlation （r=-0.518,P〈0.01）.Conclusion Plasma levels of Copeptin,NT-proBNP in CHF patients after diagnosis,the severity of judgment,evaluation has a certain significance.
