鼻内镜术后迟发性出血的临床治疗探讨

目的 探讨鼻内镜术后治疗迟发性出血所需的适当止血方式.方法 回顾性分析82例接受鼻内镜手术且术后迟发性出血的患者,根据鼻腔填塞材料不同分为:Ⅰ组,应用凡士林纱条,共25例;Ⅱ组,应用藻酸钙纤维,共31例;Ⅲ组,应用胶原蛋白海绵,共26例.比较3组的头部疼痛程度、鼻部自觉症状、出血量、黏膜水肿程度的情况.结果 与Ⅱ组、Ⅲ组相比,Ⅰ组的头部疼痛程度、鼻部自觉症状、出血量、黏膜水肿程度均明显较严重(P<0.05).而Ⅱ组与Ⅲ组之间则无明显差异(P>0.05).结论 鼻内镜术后治疗迟发性出血尽量避免使用凡士林纱条.

The clinical therapy of tardive bleeding after nasal endoscopic surgery

Objective To analysis the appropriate hemostasia methods to tardive bleeding after nasal endoscopic surgery.Methods A total of 82 patients suffered with tardive bleeding after nasal endoscopic surgery in our hospital were collected to retrospective analyses and divided into three groups,Ⅰ group （applicated vaselin pledget,25 patients）,Ⅱ group （applicated calcium alginic acid fibre,31 patients） and Ⅲ group （applicated collagen protein sponge,26 patients）.The headache degree,nosal subjective symptom,haemorrhage volume and mucosa edema degree after nasal endoscopic surgery were compared between three groups.Results Compared to Ⅱ and Ⅲgroup,the headache degree,nosal subjective symptom,haemorrhage volume and mucosa edema degree were more serious than Ⅰgroup（P〈0.05）.However,the above indexes were no different between two groups（P〈0.05）.Conclusion The hemostasia method to tardive bleeding after nasal endoscopic surgery should be avoid vaselin pledget.
