溃疡性结肠炎严重程度分级与黏附分子CD44、CD62p和CD54表达关系的研究

目的探讨溃疡性结肠炎患者炎性反应严重程度分级与黏附分子表达之间的关系。方法纳入溃疡性结肠炎患者42例,其中轻度组20例、中度组8例、重度组6例、缓解组8例,另设健康对照组30例。采用流式细胞仪检测外周血中吞噬细胞膜糖蛋白(CD44)、可溶性黏附分子P选择素(CD62p)及可溶性细胞间黏附分子-1(ICAM-1,CD54)表达情况。结果中、重度组的CD44、CD62p表达最高,轻度组、缓解组次之,健康对照组最低,各组间差异有统计学意义(P<0.01)。而中、重度组的CD54阳性表达高于轻度组(P<0.05),2组均高于缓解组及健康对照组(P<0.01),缓解组CD54阳性表达高于健康对照组(P<0.01)。结论黏附分子CD44、CD62p及CD54在溃疡性结肠炎发病过程中起重要作用,其水平高低可反映结肠组织的炎性反应严重程度,有助于指导临床治疗。

The relationship between inflammatory degree of ulcerative coliti s and the expression of CD44,CD62p and CD54

Objective To investigate the relationship between the inflammatory degree of ulcerative colitis(UC)and the expression of adhesion molecules CD44,CD62p and CD54 in peripheral blood.Methods The expressions of adhesion molecules CD44,CD62p and CD54 in peripheral blood were determined by FCM method in 42 patients with UC including 20 cases in light degree group,8 cases in moderate degree group,6 cases in severe degree group,8 cases in remission group,30 healthy subjects were enrolled as control group.Results The levels of CD44,CD62p in moderate degree group and severe degree group were highest,next light degree group and remission group,while that in control group were lowest,there were significant differences among the five groups(P<0.01).The levels of CD54 in moderate degree group and severe degree group were significantly higher than that of light degree group(P<0.05),and which in moderate degree group and severe degree group were significantly higher than those in remission group and control group(P<0.01).The positive expression of CD54 in remission group was significantly higher than that of control group(P<0.01).Conclusion CD44,CD62p and CD54 play important roles in the pathogenesis of UC,and its levels can reflex the severe degree of illness,which is helpful to guide clinical treatment for UC.
