半椎板切除显微手术治疗椎管内肿瘤

目的探讨半椎板切开入路显微切除椎管内肿瘤手术方式的优缺点。方法回顾性分析2004年1月至2006年1月经半椎板手术切除,病理证实的椎管内肿瘤80例的临床资料。结果80例病人中,75例肿瘤完全切除,其余的5例由于肿瘤经椎间孔向外生长,与周围的组织粘连紧密,行部分切除。术后随访3-9个月,平均6个月,没有发现肿瘤复发或是脊柱的不稳定。结论单侧半椎板入路显微切除椎管内肿瘤损伤小,最有利于脊柱稳定性的维持。病人手术后住院时间短,反应轻微,远期疗效好。由于手术显露局限,对手术技术要求高。

Unilateral hemilaminectomy for the microsurgical removal of intraspinal tumors

Objective To evaluate the advantages and disadvantages of unilateral hemilaminectomy for the microsurgieal removal of intraspinal tumors.Methods 80 patients with intraspinal tumors confirmed pathologically were reviewed retrospectively from 2004 to 2006.All cases were treated microsurgically using unilateral hemilaminectomy procedures.Results Tumors were totally removed in 75 patients,partially removed in the other 5 cases due to exterior growth through the intervertebral foramen and confirmed adhesion with tissue around.Postoperative following up ranged 3-9 months(mean 6 months),no recurrence and spinal instability were revealed.Conclusions Unilateral hemilamilectomy for the removal of intraspinal tumors is a mini-invasive procedure,and is beneficial to the stability of spine,with the advantages of short hospitalization time,slight postoperative reactions,good long-term outcome.But for limited exposure of operative field,the procedure requires a skilled tecnology.
