脑卒中后肩手综合征的综合干预治疗

目的:探讨综合治疗对脑卒中后肩手综合征的治疗效果。方法:对185例脑卒中后并发肩手综合征患者随机分为综合治疗组(观察组)和对照组,两组均行常规康复训练,观察组还另外采用良肢位摆放、压迫性向心缠绕、冷水热水交替浸泡等综合干预措施,治疗4周后评定疗效。结果:观察组偏瘫侧肩痛、关节活动度、水肿等与治疗前比较有明显改善,与对照组比较有显著性差异(P<0.05)。结论:脑卒中后肩手综合征的综合治疗能明显提高康复治疗效果,且有助于肩关节功能的恢复。

The integrated treatment on rehabilitation for cerebral apoplexy patients complicated with shoulder-hand syndrome

Objective:To observe the effect of hemiplegic shoulder-hand syndrone by integrated rehabilitation in stroke patients.Methods:185 stroke patients with hemiplegia were randomly divided into integrated rehabilitation group(observe group) and control group.Two groups all received routine rehabilitation.The observe group were subjected to many rehabilitation disciplinary methods such as the corrective positioning of limbs and trunk,compressive centripetal wrapping , cold-warm water marinated alternant, etc. Four weeks after treatment,the efficacy was evaluated.Results:The efficacy on the observe group was significantly higher than in control group(P<0.01).Compared with that before treatment, the VAS score on shoulder pain,ROM of joint,edema of affected limb were obviously improved on the observation group after treatment,and the VAS score also had a significant difference between the two groups after treatment(P<0.05).The FMA and BI scores in the two groups was markedly improved after treatment(P<0.05 ),especially in observation group(P<0.05).Conclusion:the integrated rehabilitation has significant effectiveness on rehabilitation for stroke patients complicated with shoulder-hand syndrome,and is helpful to recover the function of shoulder joint.
