多层螺旋CT在急性阑尾炎诊断中的应用

目的：探讨多层螺旋CT（MDCT）技术诊断急性阑尾炎的临床应用价值。方法：收集35例临床手术确诊为急性阑尾炎的患者,利用薄层多平面重组,总结分析其CT征象,并将CT征象与手术及病理结果进行对照。结果：35例阑尾炎中,本组Ⅱ级6例,Ⅲ级13例,Ⅳ级7例,Ⅴ级9例。MDCT征象分直接征象和间接征象,直接征象包括阑尾增粗、增大,阑尾壁增厚,阑尾内见粪石影及阑尾腔内积液。间接征象包括阑尾周围炎、阑尾周围脓肿和腹腔游离气体。结论：急性阑尾炎有典型的CT表现,MDCT三维重组及图像后处理技术明显提高对阑尾炎的诊断能力,对阑尾炎有较高的诊断价值。

Application of multi-detector computerized tomography in the diagnosis of acute appendicitis

Objectives: To investigate the clinical value for multi-detector computerized tomography in the diagnosis of acute appendicitis. Methods: Thirty five acute appendicitis patients were collected, used lamella multiple-level recombination, summarized the symptom of CT, and compared the operative findings and pathologic results were assessed. Result: In 35 cases, we found 6 cases in grade Ⅱ,13 cases in grade Ⅲ, 7 cases in grade Ⅳ and 9 cases in grade Ⅴ. The MDCT sign included the direct and indirect sign characteristics. The direct sign showed with the image of appendix and its wall thickening and increasing, bezoars or (and) effusion in appendix cavity. The indirect sign showed with periappendicural inflammation and abscess, intraperitoneal free gas. Conclusion: Acute appendicitis have typical CT image. The three-dimensional re-image and post-processing techniques of MDCT can efficiently and highly improve the diagnostic ability to the acute appendicitis.
