单孔悬吊法腹腔镜阑尾切除术的应用体会

目的：探讨单孔悬吊法腹腔镜阑尾切除术（laparoscopic appendectomy,LA）的可行性,并总结治疗体会。方法：为43例阑尾炎患者施行单孔悬吊法LA。经脐部单孔分别置入腹腔镜和操作钳,找到阑尾后,从右下腹置入细铜丝悬吊起阑尾根部,在腹腔内完成阑尾切除,经脐孔拖出阑尾。结果：手术时间25-97min,平均36min,1例中转开腹,1例经两孔法切除。术后1例发生脐孔感染,无其他并发症发生。所有患者均痊愈出院。平均住院4.5d。结论：单孔悬吊法LA操作简单、有效、可行,扩大了单孔LA的适应证,并且有很好的美容效果。

Clinical application with laparoscopic appendectomy in single-port and suspension method

Objective:To discuss the feasibility with laparoscopic appendectomy(LA) in single-port and suspension.Methods:Forty-three patients with appendicitis were performed LA with single-port and suspension method.Laparoscope and operating forceps were separately introduced into abdomical cavity through the single umbilical port.After the appendix was identified,the root was suspended with a piece of fine copper wire and the appendectomy was performed.Results:All of 43 patients were successfully performed LA.The average operative time was 36min(range 25-97min).One case was converted to open appendectomy.One case was with double-pore method.One case suffered from umbilical port infection.There were no other operative complications.The mean postoperative hospitalization were 4.5 days.Conclusions:The laparoscopic appendectomy with single-port and suspension method is a feasible,effective and safe technique.It expands the indications with one-trocar laparoscopic appendectomy and has a good cosmetic effect.
