同种异体骨移植在基层医院的应用研究

目的:探讨同种异体骨在修复骨缺损促进骨融合中的治疗效果。方法:1994年3月~2005年4月,笔者对创伤、良性肿瘤、瘤样病变、脊柱融合、人工关节翻修患者89例,分别采用酒精储骨(A组)或山西太原医用组织库提供的冻干骨(B组)单独或合并自体骨移植治疗。全部获得随访,其中A组40例,B组49例。术后记录感染、排异反应、骨不连的发生率及平均骨愈合时间。结果:A组感染1例(2.5%)、排异反应2例(5%)、骨不连2例(5%)、平均骨愈合时间(3.5±0.46)个月;B组感染1例(2%)、排异反应3例(6.1%)、骨不连2例(4.1%)、平均骨愈合时间(3.7±0.48)个月。两组差异无显著性(P>0.05)。结论:酒精储骨和冻干骨无明显的排异反应,用于修复骨缺损促进骨愈合可获得满意的临床效果,酒精储骨因取材、保存方便、价格低廉,更适合在基层医院开展。

Application of Allogeneic Bone Graft in Basic Level Hospital

Objective:To research the therapeutic effect of allogeneic bone graft used in the reparation of bone loss and bone fusion.Methods:From 1994 March to 2005 April,there are 89 cases including trauma,benign tumor,tumor-like pathological changes,spine fusion,joint replacement patients.They were divided into 2 groups: group A alcohol stored bone and group B freeze-dried bone(gotten from Taiyuan Shanxi medical bank).They were treated combined with allogeneic bone graft.All of the cases were followed up,group A 40 cases,group B 49 cases.The infection and rejection rate,nonunion rate and average bone heal time were recorded.Results: In group A the infection arose in a case(2.5%),the rejection and nonunion occurred in 2 cases,separately(5% each),and the average bone heal time was(3.5±0.46)months.In group B the infection,rejection and nonunion occurred in one,3 and 2 cases,separatly,and the average heal time was(3.7±0.48)months.There were no significant difference in two groups(P>0.05).Conclusion:The alcohol stored bone and freeze-dried bone have no rejection,all of them can be used in the reparation of the bone loss and can accelerate the bone healing.Alcohol stored bone is a kind of common,easy to conservation and cheap materials.So it is fit for the generalization in the basic level hospital.
