微创锁定加压钢板内固定治疗胫骨远端骨折54例

摘    要:目的探讨胫骨远端骨折的手术时机和手术方法。方法 2007年8月~2010年11月收治胫骨远端骨折54例,按AO分型,A型18例:B型24例,C型12型。所有病例均先行跟骨牵引,待皮纹征出现后,行经皮微创锁定加压钢板内固定。结果 51例获得随访,随访率94.4%,随访7~24个月,平均随访(13±4.5)个月。骨折愈合时间3~6个月,按Tornetta判断标准,优36例,良11例,可4例,差0例;优良率92.2%。结论采用先行跟骨牵引,二期经皮微创锁定钢板内固定治疗胫骨远端骨折可以最大限度减轻对软组织的损害和对骨膜血运的破坏,有利于切口和骨折的愈合。

Abstract:Objective To evaluate the clinical the outcome of minimally invasive locking compression plate in treating distal tibial fractures Methods From Aug 2007 to Nov 2010,54 distal tibial fracture patients were treated.According to AO classification,type A in 18 cases,type B in 24 cases,type C in 12 cases.All the patients were treatd with skeletal traction before operation.When limb swelling was disappeared,the distal tibial fractures were treatd with percutaneous minimally invasive locking compression plate(LCP).Rusults 51 patients were followed-up.The mean follow-up period was 13??5 months with a range from 7 to 24 months.Bone healing was achieved 3 to 6 months after operation.According to Tornetta scoring system,there were excellent in 37 cases,good in 11 cases,normal in 4cases,and no bad case.The excellent and good rate was 92.2%.Conclusions After skeletal traction,the LCP represents a superiority in treating the distal tibial fractures,with advantage of protecting parenchyma and periosteum,facilitating incision and fractures healing.
