高频超声评价冠状动脉狭窄与颈动脉内-中膜厚度及面积的关系

摘    要:目的 :对颈总动脉内 -中膜厚度 (IMT)与程度不同的冠状动脉粥样硬化性狭窄之间的关系进行再探讨 ,比较 IMT与颈总动脉内 -中膜横切面面积 (IMCSA)对冠状动脉狭窄的预测价值。方法 :55例行冠脉造影的患者分成非冠心病组、冠状动脉 1支病变组和多支病变组 ,运用高频超声技术检测三组患者颈总动脉 IMT和斑块 ,计算 IMCSA。比较三组间 IMT和 IMCSA的差异 ,比较 IMT和 IMCSA预测冠状动脉狭窄的敏感性和特异性。结果 :冠状动脉 1支和多支病变组间 IMT无显著差异 ,IMCSA和斑块面积在多支病变组显著增高 (P<0 .0 1)。用 IMCSA来预测冠状动脉粥样硬化狭窄能更好地兼顾敏感性和特异性。结论 :颈总动脉 IMCSA较 IMT能更好地预测冠状动脉粥样硬化狭窄的发生 ,区分不同程度冠状动脉狭窄的能力更强

Abstract:Objective:To further investigate the relationship between carotid intima-media thickness(IMT) and various degree of coronary atherosclerotic stenosis with high frequency ultrasound and to campare the predicting value of IMT or carotid intima plus medial cross-sectional are a(IMCSA) for coronary stenosis Methods:Fifty-five cases who accepted coronary angiograp hy were divided into 3 groups:group normal,1 vessel and multi-vessel groups W e measured the IMT and IMCSA and detected plaques in carotid artery in all patie nts The difference of them among 3 groups and the sensitivity and specificity w ere calculated Results:The IMT was not different statistically between 1 and multi-vessel groups while IMCSA and plaque area were significantly increase d in multi-vessel group than those in 1 vessel group ( P <0 01) We could m ake sensitivity and specificity more definite if we predicted coronary stenosis by IMCSA Conclusions:Carotid IMCSA can predict and reflect the degr ee of coronary atherosclerotic stenosis more efficant than IMT simply
