小剂量镇静药物咪哒唑仑对老年冠状动脉介入患者的抗焦虑作用

目的:研究小剂量(0.1mgkg)镇静药物咪哒唑仑片(多美康片)对老年人在冠状动脉介入术中的抗焦虑作用。方法:随机、前瞻、对照性研究103例首次接受冠状动脉介入术的老年患者,平均年龄(66.6±8.4)岁,其中A组(多美康组)43例,B组(安定组)60例,分别于介入术前30min口服多美康片7.5mg(0.1mgkg)或安定片2mg,采用汉密尔顿焦虑评分量表评价焦虑的程度。术中监测股动脉血压和心电活动,记录术中的缺血事件和心律失常。结果:服药后多美康组无论躯体因素评分,还是心理因素评分均明显低于安定组,术中作为焦虑症状临床表现之一的血压、心率亦明显低于安定组。结论:小剂量咪哒唑仑可明显抑制老年患者对冠状动脉介入手术产生的焦虑和恐惧心理,并抑制由于焦虑和心理紧张而导致的血压、心率增加。

The use of the lower dose of midazolam in the eldly patients with coronary catheterization

Objective:To study the anti-angentic effects of the lower dosage of midazolam on the eldly with undergoing coronary catheterization.Method:This was a randomly, control, prospectly, the 103 old patients with primary undergoing coronary catheterization were studied,whose mean age were (66.6±8.4) years. the lower dosage 7.5 mg midazolam(0.1 mg/kg) was given in the Midazolam_group including 43 cases about 30 minutes before the coronary catheterization, 2 mg diazepam was given in diazepam_group including 60 patients about 30 minutes before the procedure. the femoral blood pressure,heart rate, arrythmia, and ischemic events were recorded. The effects of anxiety were evaluated with HAMA.Result:1. Findings indicated the body anxiety sub_score,psychic anxiety sub_score and total scores of midazolam are less than those of the diazepam group. 2.The mean blood pressure and heart rate of the midazolam group were obviously lower than those of the diazepam group.Conclusion:Responsing to the coronary catheterization,the lower dosage of midazolam may improve the anxiety and fear of old paitients and decreas the increasing of blood pressure and heart rate due to the anxiety and tension moods.
