摘    要:目的探讨轻度认知功能障碍及阿尔茨海默病患者甲状腺功能的变化。方法用放射免疫法检测轻度认知功能障碍(mildc ognition impairment,MCI)、阿尔茨海默病(Alzheimer's disease,AD)组,及正常老年人组三碘甲状腺原氨酸(triiodo-thyronine,T3)、T4甲状腺素(tetraiodothyronine,T4)、游离三碘甲状腺原氨酸(free triiodothyronine,FT3)、FT4游离甲状腺素(free thyroxine,FT4)、促甲状腺激素(thyrotropin,TSH)水平。结果MCI患者T3的水平明显降低〔(0.51±0.14)μg/L〕,AD患者T3的水平明显降低〔(0.54±0.11)μg/L〕,与对照组〔(0.74±0.17)μg/L〕比较,差异有统计学意义(均P〈0.01);MCI及AD患者的FT3水平也明显降低〔(1.62±0.34)pmol/L,(1.55±0.32)pmol/L〕,与对照组〔(1.94±0.46)pmol/L〕比较,差异有统计学意义(均P〈0.01);与对照组TSH〔(2.94±2.12)mU/L〕比较,MCI患者TSH水平〔(2.62±1.98)mU/L〕无明显变化(P〉0.05),AD患者TSH水平明显降低〔(1.54±0.88)mU/L〕(P〈0.01)。结论MCI及AD患者甲状腺功能降低,TSH在MCI向AD的转变过程中可能有临床参考价值。

Abstract:Objective]To explore the alterations of thyroid function in patients with mild cognition impairment and Alzheimer disease.Methods]The levels of serum triiodothyronine(T3),tetraiodothyronine(T4),firee triiodothyronine(FT3),free thvroxine(FT4),and thvrotroDin(TSH) were measured by radioimmunoassay in mild cognition impairment(MCI) group,Alzheimer disease (AD) group,and normal elderly group.Results]The levels of T3 significantly decreased in MCI group 〔(0.51±0.14) μg/L〕,and that of AD group also decreased obviously〔(0.54±0.11) μg/L〕,comparing with the control group 〔(0.74±0.17) μg/L〕,the difference was significant (both P0.01).The levels of FT3 decreased in MCI group and AD group 〔(1.62±0.34) pmol/L,(1.55±0.32)pmol/L〕 as compared with control group 〔(1.94±0.46 pmol/L〕,the difference was significant (both P0.01);compared with controls〔(2.94±2.12) mU/L〕,the level of TSH in MCI group 〔(2.62±1.98) mU/L〕 did not change significantly (P0.05),while the level of TSH in AD group〔(1.54±0.88 )mU/L〕 decreased significantly (P0.01).Conclusion]Thyroid function reduces in MCI and AD patients,the TSH level implies a possible clinical value in MCI to AD transformation.
