肾移植后巨细胞病毒肺炎的X线和CT表现

目的: 提高X线胸片和CT诊断肾移植后巨细胞病毒(CMV)肺炎的水平.材料和方法: 回顾性分析11例肾移植后CMV患者的X线胸片和CT表现.结果: 肾移植后CMV肺炎8例X线胸片表现为磨玻璃影和实变影,3例胸片正常;CT表现为双肺弥漫性病变,磨玻璃影最为常见,并与其它影像学表现合并存在,主要与实变影和多发性小结节样影合并存在;磨玻璃影在所有患者均表现为两侧肺分布.10例患者有多发性小结节样影;8例患者有多发实变影;5例并发有少量胸腔积液和小叶间隔增厚.结论: 肾移植后CMV肺炎的CT表现有一定特点,结合临床可提示诊断.

The X-ray and CT Appearances of Cytomegalovirus Pneumonitis after Kidney Transplant

Purpose: To improve chest radigrcophic and CT diagnosis in cytomegalovirus pneumonia after kidney transplant. Materials and Methods: The chest radiogrqphic and CT appearances of 11 patients with cytomegalovirus prceumonitis after kidney transplomt were retrospectively analysed. Results: Bilateral ground-glass opacity and consolidation on the chest X-ray films were seen in 8 patients, and the chest X-ray films were normal in 3 patients. Bilateral abnormalities on CT scans were seen in all patients. The most frequent CT appearance was ground-glass opacity, seen in all patients. It was mixed with other patterns, mainly areas of small nodules and consolidation. The areas of ground-glass opacity were distributed bilaterally in all patients. Small nodules were seen in 10 patients. The area of consolidation was bilateral in 8 patients. Bilateral areas of thickened interlobular septa were present in 5 patients. Pleural effusion was seen in 5 patients. Conclusion: The CT manifestations of cytomegalovirus pneumonitis after kidney transplant have some characteristics,associated with clinic data, can suggest the diagnosis of cytomegalovirus prenoronitis.
