颅内血肿微创清除术联合中药分期治疗高血压脑出血的临床疗效及其安全性

目的探讨颅内血肿微创清除术联合中药分期治疗高血压脑出血的临床疗效及其安全性。方法选取绥阳中医院脑科中心2017年1月—2018年7月收治的高血压脑出血患者82例,采用随机数字表法分为对照组和观察组,各41例。两组均采用颅内血肿微创清除术治疗,对照组在此基础上给予西医常规治疗,观察组在此基础上给予中药分期治疗。比较两组临床疗效,治疗前后中国脑卒中临床神经功能缺损程度量表(CSS)评分、美国国立卫生研究院卒中量表(NIHSS)评分、Barthel指数,术后并发症、首次下床活动时间、住院时间。结果观察组总有效率高于对照组(P<0.01)。治疗前两组CSS评分、NIHSS评分、Barthel指数比较,差异无统计学意义(P>0.05);治疗后观察组CSS评分、NIHSS评分低于对照组,Barthel指数高于对照组(P<0.05)。观察组术后并发症发生率低于对照组,首次下床活动时间、住院时间短于对照组(P<0.01)。结论颅内血肿微创清除术联合中药分期治疗高血压脑出血的临床疗效确切,可有效促进患者神经功能及自理能力恢复,且安全性高。

The clinical efficacy and safety of minimally invasive removal of intracranial hematoma combined with traditional Chinese medicine staging for hypertensive cerebral hemorrhage

Objective To explore the clinical effect and safety of minimally invasive removal of intracranial hematoma combined with traditional Chinese medicine staging for hypertensive cerebral hemorrhage.Methods A total of 82 cases of patients with hypertensive cerebral hemorrhage were selected from January 2017 to July 2018 in Brain Center of Suiyang Hospital of Traditional Chinese Medicine,which were divided into control group and observation group,41 cases each of group.Both groups were treated with minimally invasive removal of intracranial hematoma,the control group was treated with conventional western medicine on this basis,and the observation group was treated with Chinese medicine by stages on this basis.The clinical efficacy,CSS score,NIHSS score,Barthel index before and after treatment,postoperative complications,first time out of bed activity and hospitalization time between the two groups were compared.Results The total effective rate of the observation group was higher than the control group(P<0.01).There was no significant difference in CSS score,NIHSS score and Barthel index between the two groups before treatment(P>0.05);after treatment,CSS score and NIHSS score in the observation group were lower than the control group,and Barthel index was higher than the control group(P<0.05).The incidence rate of postoperative complications in the observation group was lower than the control group,and the first time out of bed activity and hospitalization time were shorter than the control group(P<0.01).Conclusion Minimally invasive removal of intracranial hematoma combined with traditional Chinese medicine staging for hypertensive cerebral hemorrhage has definite clinical effect,it can effectively promote the recovery of neurological function and self-care ability of patients,and with high safety.
