摘    要:目的探讨高分子量细胞角蛋白34βE12、肌上皮标记物p63、细胞角蛋白CK5/6、平滑肌肌动蛋白α-SMA在前列腺基底细胞中的表达及意义。方法采用免疫组织化学SP法检测,观察34βE12、p63、CK5/6、α-SMA在前列腺增生(PH)、低级别上皮内瘤变(LGPIN)、高级别上皮内瘤变(HGPIN)、前列腺癌(Pca)中的表达。结果 30例PH组标本34βE12、p63、CK5/6大多阳性表达,10例LGPIN组多(++)表达,15例HGPIN组多(+)表达,Pca组大多阴性表达。Pca组与PH组、LGPIN组、HGPIN组比较差异有统计学意义(P〈0.05)。α-SMA基底细胞少量表达,Pca组与PH组、LGPIN组、HGPIN组比较差异均无统计学意义(P〉0.05)。结论前列腺基底细胞表达34βE12、p65、CK5/6,p65优于34βE12,α-SMA多阴性表达。在前列腺标本中联合检测34βE12、p65、CK5/6有助于对前列腺癌及其他不同病变的鉴别诊断。

Abstract:Objective To explore the expression and significance of high molecular weight eytokeratin (34/3 E12), myoepithelium marker (p65), eytokeratin (CK5/6) and smooth muscle actin (w-SMA) in the basic cells of prostate. Methods Immunohistoehemieal SP method was performed to detect the expression of 34βE12, p65, CK5/6 and α-SMA in prostatic hyperplasia (PH), low-grade intraepithelial neoplasia (LGPIN), high-grade intraepithelial neopla- sia (HGPIN) and prostate cancer (Pca).Results In 30 PH samples, 34/3E12, p65 and CK5/6 were expressed (++) in 10 LGPIN samples, (+) in 15 HGPIN samples and negative in Pca samples. There were significant statistical differenees between Pea group and PH, LGPIN and HGPIN groups. Little α-SMA was expressed in basal cells and there was no significant statistical difference between these groups (P〉0.05). Conclusion 34/3E12, p65 and CK5/6 were expressed in prostate basic cells and p65 was expressed more than 34fiE12. α-SMA was negative in most of the prostatic basic cells. Joint detection of 3413E12, p65 and CK5/6 was helpful for the differential diagnosis of prostate cancer.
