研究糖化血红蛋白测定在妊娠糖尿病诊断和监测中的临床意义

目的研究糖化血红蛋白（HbAlc）在妊娠期糖尿病（GDM）的预防、诊断和监测中的临床意义。方法根据临床诊断筛选200例21～30孕周、年龄19～40岁的孕妇，以IADPSG新的推荐标准，分为正常妊娠组100例和妊娠糖尿病组（GDM组）100例。两组分别进行FPG、OGTT和HbAlc测定，对所得数据进行统计学分析，比较两组FPG、OGTr和HbAlc。结果GDM组FPG、OGTr和HbAlc明显高于正常妊娠组，差异具有统计学意义（P=0．000）。GDM组的HbAlc阳性率高达82．0％，而正常妊娠组HbAlc的阳性率为6．0％，差异具有统计学意义（P〈0．05）。结论HbAlc在妊娠期糖尿病诊断中具有很高的阳性率，联合FPG和OGTY对血糖监测和治疗具有重要的临床价值。

Determination of glycosylated hemoglobin for diagnosis and monitoring of gestational diabetes mellitus

Objective To study the clinical significance of glycosylated hemoglobin （HbAlc） in preventing, diagnosing, and monitoring gestational diabetes mellitus （GDM）. Methods 200 women, 21-30 weeks pregnant and aged 19-40, were selected and then were divided into a normal pregnant group and a GDM group according to the new standard of IADPSG, 100 for each group. FPG, OGTT, and HbAlc were determined in both groups, and their results were statistically analyzed and compared. Results FPG, OGTF, and HbAlc were obviously higher in the GDM group than in the control group, with statistical differences （P=0.000）. The positive rate of HbAlc was 82.0% in the GDM group and was 6.0% in the control group, with a statistical difference （P 〈 0.05）. Conclusions HbAlc has a very high positive rate in GDM. Determination of HbAlc, FPG, and OGTF has significant clinical value in GDM monitoring and treatment.
