心室晚电位与高血压左室肥大伴室性心律失常的关系分析

目的　探讨心室晚电位与左室肥大伴室性心律失常之间的关系。方法　通过观察 67例合并左室肥大及 113例无合并左室肥大高血压患者的心室晚电位和心电工作站检查 ,所获数据经 χ2 检验。结果　心室晚电位阳性率 13.9% ,合并或不合并左室肥大分别为 2 6.8%和 6.2 % (P <0 .0 1) ,室性心律失常检出率为 82 .3%和 38.4 % (P <0 .0 1) ,心室晚电位阳性左室肥大伴室性心律失常 ,复杂性心律失常及室性心动过速检出率( 10 0 % ,61.1% ,2 7.7% ) ,均高于心室晚心电位阴性的左室肥大者 ( 79.6,4 2 .9% ,12 .3% ,P <0 .0 5) ,及无左室肥大的心室晚电位阳性 ( 75.0 % ,2 5.0 % ,12 .5,P <0 .0 5) ,结论　心室晚电位对高血压左室肥大伴室性心律失常 ,尤其是复杂性室性心律失常有一定的预测价值

Relationship Between Ventricular Late Potential and Hypertensive Left Vertricular Hypertrophy Associated with Ventricular Arrhythmia

Objective To study the relationships between ventricular late potential and hypentensive left venticular hypertrophy associated with ventricular arrhythmia. Methods 67 patients with hypertension associated with left ventricular hypertrophy and 117 patients with hypertension but without left ventricular hypertrophy were examiedwith ventricular late potential and electrocardial workshop. Results The positive rates of ventricular late potential wwere 13.9%, the positive rates of ventricular late potential associated with leftventricular hypertrophy but without left ventricular hypertrophy were 26.8% and 6.2%,respectively (P<0.01). The positive rates of ventricular arrhythmia were 82.3% and 38.4% respectively ( P<0.01). Thepositive rates of ventricular arrhythemia and complicated arrhythmia and ventricular tachycardia ( 100%, 61%, 27.7%) associated with left ventricular hypertrophy in abnormal ventricular late potential were higher than the rates associated with left ventricular in abnormal ventricular potential (79.6%,42.9%,12.3%,P<0.05) and than the rates without left ventricular hypertropy in abnormal ventricular late potential (75%,25%,12.5%,P<0.05).Conlusion This study suggests that ventricular late potentical has some value to predict hypermtensive left ventricular hypertrophy associated with ventricular arrhythmia, especialy complicated ventricular arrhythemia.
