急性冠脉综合征与脂蛋白相关磷脂酶A2的相关性

目的 探讨急性冠脉综合征患者血清脂蛋白相关磷脂酶A2（Lp-PLA2）的水平及其对预后的临床价值.方法 入选急性冠脉综合征（ACS）患者112例,分为不稳定型心绞痛（UA）组、急性心肌梗死（AMI）组和非ST段抬高型心肌梗死（NSTEMI）组,其中UA组根据危险度分层分为低危、中危和高危组;对照组78例.所有患者均行冠状动脉造影检查,采用Gensini积分方法对各支冠状动脉病变程度进行评定,用ELISA方法测定各组Lp-PLA2水平,进行对比分析.结果 ACS各组患者血清Lp-PLA2水平均显著高于对照组（P＜0.01）;AMI组Lp-PLA2水平显著高于UA组（P＜0.01）;高危、中危组Lp-PLA2水平均高于低危组（P＜0.01,P＜0.05）,且三组相对应的冠脉评分也随着危险度的升高而增加（P＜0.01）.结论 Lp-PLA2水平可作为预测ACS病情严重程度及预后的重要生化指标之一.

Acute coronary syndrome and the correlation of lipoprotein associated phospholipase A2

Objective To investigate the clinical value of prognosis and serum lipoprotein-associated phospholipase A2（Lp-PLA2） level in patients with acute coronary syndrome（ACS）.Methods According to the results of acute coronary syndrome,the 112 patients were divided into unstable angina （UA）,acute myocardial infarction（AMI） and non-ST-segment elevation myocardial infarction（NSTEMI） group.UA group according to risk stratification were divided into low-risk group,moderate group and high-risk group.The control group（n=78）.All patients were performed coronary angiography examination,the Gensini score method to assess the degree of each branch of coronary artery lesions.The level of Lp-PLA2 was detected by ELISA in all groups and the results were compared and analyzed.Results Serum Lp-PLA2 level in ACS group was significantly higher than that in controls （P＜0.01）,the level of Lp-PLA2 in AMI group was significantly higher than that in UA group（P＜0.01）,the level of Lp-PLA2 were in high risk and the moderate group higher than in low-risk group,and the corresponding with the coronary artery lesion score also increased with the increasing the risk stratification.Conclusion Serum LpPLA2 level can be used as important biochemical indicators to predict disease severity and prognosis of the ACS.
