支气管肺泡灌洗在ICU机械通气患者中的应用

目的探讨支气管肺泡灌洗治疗呼吸机相关性肺炎的临床价值。方法对42例机械通气并发呼吸机相关性肺炎患者进行支气管肺泡灌洗,观察灌洗后临床疗效及灌洗后血气分析和呼吸力学指标变化。结果所有患者灌洗后临床症状改善,PaO2上升、PaCO2下降、气道阻力下降及肺动态顺应性改善,差异有统计学意义（P〈0．05）。结论支气管肺泡灌洗对于ICU机械通气并发呼吸机相关性肺炎患者是安全、有效的,值得在临床上推广应用。

Application study on bronchoalveolar lavage in the patients of mechanical ventilation in ICU

Objective Discussion bronchoalveolar lavage treatment of ventilator-associated pneumonia clinical value.Methods 42 cases of respiratory complication of mechanical ventilation-associated pneumonia in patients with bronchoalveolar lavage, to observe the clinial effect and indicator changes of blood gas analysis and respiratory mechanics after lavage. Results After lavage, all patients with clinical symptoms improve,PaO2 increased, PaCO2 decreased, airway resistance decreased and lung decline in dynamic compliance improved significantly（ P 〈 0.05）. Conclusion Bronchoalveolar lavage is a safe and effective for complicated mechanical ventilation ICU ventilator-associated pneumonia patients. Worthy of clinical application.
