针刺对脑出血急性期神经功能缺损改善程度时效关系的临床研究

目的：观察针刺对脑出血急性期神经功能缺损改善程度的时效关系，确定针刺治疗脑出血介入的最佳时间。方法：300例脑出血急性期患者按照随机数字表法随机分为对照组及治疗组，对照组60例，给予脑出血常规治疗，治疗组在此基础上加用针刺治疗，按针刺介入时间分为发病24小时针刺组、72小时针刺组、1周针刺组及2周针刺组，每组均60例。结果：各治疗组神经功能缺损评分差值较对照组均有统计学差异（P〈0．05）；治疗组之间比较，本项指标改善程度随针刺介入的时间先后呈正相关，具有统计学差异（P〈0．05），且以发病后24小时针刺组疗效最为显著。结论：针刺在脑出血急性期的介入时间越早，对患者神经功能缺损改善程度越显著。故脑出血后应尽早应用针刺治疗。

Clinical Research on Time-Effect of Acupuncture Treatment for the Improvement of Neurological Deficit after A- cute Intracerebral Hemorrhage

Objective： To research the time-effect relationship of acupuncture treatment for neurological deficit improvement after acute intracerebral hemorrhage （ICH） and conform the most suitable time of acupuncture treatment for acute intracerebral hemorrhage. Methods： 300 cases were randomly divided into 5 groups such as： the controlled group, 24 hour acupuncture group, 72 hour acupuncture group, 1 week acupuncture group and 2 week acupuncture group. Each team contained 60 cases. Cases received the conventional treatment in the controlled team. Others received acupuncture treatment as well as conventional treatments in different time ： 24 hours after ICH, 72 hours after ICH, 1 week after ICH and 2 weeks after ICH. Before and after these treatments, we measured the cerebral blood flow and statistics the outcome. Results： There were statistical differences of the neurological deficit improvements between the treatment group and controlled group （ P 〈 0. 05 ）. For the treatment teams, there was a upward trend of the neurological deficit improvement as early as acupuncture treatment applied （P 〈 0. 05）. The ca- ses had the apparent improvement of neurological deficit in the 24 hour group. Conclusion： The earlier of the acupuncture treat- ment applied in ICH, the more apparent of the improvement inneurological deficit. So acupuncture treatment shall be applied in ICH as soon as possible.
