阻塞性睡眠呼吸暂停低通气综合征伴高血压患者血脂和血清高敏C-反应蛋白、脑钠素水平的变化及其临床意义

摘    要:目的:探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)伴高血压患者血脂和血清高敏C-反应蛋白(hs-CRP)、脑钠素(BNP)水平的变化及其临床意义。方法采用生化法、免疫比浊法和化学发光免疫分析测定了31例单纯OSAHS患者、36例OSAHS伴高血压患者和60名正常对照组血脂以及血清hs-CRP和BNP水平,并进行了对比性分析。结果单纯OSAHS患者血清TC和LDL-C水平正常(P均>0.05),而血清TG和HGL-C水平降低(P均<0.05);OSAHS伴高血压患者血清TG和HDL-C明显降低(P均<0.01),并随OSAHS伴高血压患者血压增高的严重程度而降低愈明显。单纯OSAHS患者血清hs-CRP和BNP水平明显增高(P<0.01,P<0.05);OSAHS伴高血压患者血清hs-CRP和BNP水平非常明显增高,且随血压增高的严重程度而增高愈明显。结论 OSAHS伴高血压患者的特点是血清TG和HDL-C水平明显降低,并随血压增高的严重程度而降低,血清hs-CRP和BNP水平明显增高,且随血压增高的严重程度而增高,并可能参与疾病的病理生理过程。

Abstract:Objective To explore the clinical significance of change of blood lipid, serum high sensitive C-reaction protein (hs-CRP) and brain natriuretic peptide (BNP) levels in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) complicated with hypertension. Methods The blood lipid, serum hs-CRP and BNP levels were determined in 31 simple OSAHS patients, 36 OSAHS complicated with hypertension patients and 60 healthy controls by biochemistry, immune turbidmetry and chemiluminescence immune method as well as done compared analysis. Results The serum TG and LDL-C levels were normal (P all〉0.05) in simple OSAHS patients, but the serum TG and HDL-C levels decreased (P all〈0.05). The serum TG and HDL-C levels significantly decreased (P all〈0.01) in OSAHS complicated with hypertension patients and decreased with hypertension severity. The serum hs-CRP and BNP levels significantly increased in simple OSAHS patients (P〈0.01, P〈0.05), very significantly increased in OSAHS complicated with hypertension patients (P all〈0.01), and increased with hypertension severity. Conclusion The feature of OSAHS complicated with hypertension were that the serum TG and HDL-C levels significantly decreased and decreased with hypertension severity, but the serum hs-CRP and BNP levels significantly increased and increased with hypertension severity, which play a part in the pathogenesis of OSAHS.
