急性ST抬高型心肌梗死患者血清白细胞介素-27浓度检测的临床意义

目的 探讨白细胞介素-27（interleukin-27,IL-27）在急性ST段抬高型心肌梗死（ST-segment elevation myocardial infarction,STEMI）患者不同冠状动脉病变程度中的浓度变化及临床意义.方法 181例STEMI患者采用酶联免疫吸附法（ELISA）测血浆IL-27等指标的浓度.应用选择性冠状动脉造影确定冠状动脉狭窄程度,根据造影结果将患者分为单支病变组,双支病变组,三支病变组.结果 STEMI组血浆IL-27浓度显著高于正常组,差异有统计学意义（P＜0.01）.三支病变组在入院当时,1d、2d和7d后血浆IL-27浓度均显著高于单支病变组和双支病变组,差异有统计学意义（P＜0.01）;双支病变组在入院时、2d和7d后血浆IL-27浓度高于单支病变组,差异有统计学意义（P＜0.01）.随着冠状动脉病变程度的加重,血清肌钙蛋白Ⅰ、高敏C反应蛋白、IL-27浓度逐渐升高,差异有统计学意义（P＜0.05）.结论 STEMI患者中血浆IL-27浓度明显增高,而且与冠状动脉病变程度呈正相关.

Detection and clinical significance of plasma IL-27 in patients with acute ST-segment elevation myocardial infarction

Objectives To explore the clinical significance of plasma concentration of interleukin-27 （IL-27） in patients with ST-segment elevation myocardial infarction （STEMI） of different degrees of vascular lesions.Methods Plasma concentrations of IL-27 in 181 patients with STEMI were measured by enzyme-linked immunosorbent assay （ELISA）.Degrees of coronary artery stenosis were detected by selective coronary angiography.According to the imaging results,the patients were divided into single lesion group,double lesions group and triple lesions group.Results Plasma concentration of IL-27 in STEMI group was significantly higher than that in normal group （P〈0.01）.Plasma concentrations of IL-27 in triple lesions group on admission,day 1,2 and 7 were significantly higher than those in single lesion group and double lesions group （P〈0.01）.Plasma concentrations of IL-27 in double lesions group on admission,day 2 and 7 were obviously higher than those in single lesion group （P〈0.01）.Concentrations of cardiac troponin Ⅰ,high-sensitivity Creactive protein and IL-27 gradually increase as degrees of coronary artery stenosis were more serious （P〈0.05）.Conclusions Patients with STEMI have higher plasma concentration of IL-27.Plasma concentration of IL-27 is positively related to the degree of coronary lesions.
