| 摘 要： | 目的:比较研究劈离式肝脏移植两种主要劈离方式一经典劈离方式和改良劈离方式的差异,以选择一种安全易行的方式。方法:取５８例福尔马林固定成人尸肝和８例新鲜肝脏进行劈离肝移植的应用解剖学研究。分别采用经典劈离方式和改良劈离方式,比较肝脏各段动脉血供、静脉回流及胆管的损伤情况。结果:本组６６例肝脏６６．７%Ⅷ段、１００%Ⅴ段(部分或全部)通过肝中静脉回流。采用经典劈离方式标本中,右半植肝Ⅴ、Ⅷ段静脉回流均被破坏。结论:改良劈离方式比经典劈离方式更安全可靠,在供肝大小适合的情况下应成为临床劈离式肝脏移植的首选术式。

| Abstract: | Objective:To select a safe and feasible method of splitting liver transplantation. Methods:Fifty-eight antiseptic liver specimens and eight fresh livers were divided into two groups. One group was performed classical splitting and the other group was performed modified splitting. The lesion of artery supply and vein drainage and bile duct of all segments of liver were compared. Results: Among sixty-six livers,66.7% of the eighth segment and 100% of the fifth segment(all or some) drained to middle hepatic vein. In classical splitting group, the vein drainage of five and eight segment of right graft was destroyed. Conclusion: Modified splitting is more safe and reliable than classical splitting,so it is the first choice when the size of donor liver is suitable. |
