肾移植受者应用吗替麦考酚酯与雷帕霉素的成本-效果分析

目的对雷帕霉素或吗替麦考酚酯联合他克莫司、泼尼松方案用于肾移植受者进行成本-效果分析。方法选择肾移植术后免疫抑制方案为雷帕霉素+他克莫司+泼尼松患者60例(观察组),吗替麦考酚酯+他克莫司+泼尼松患者60例(对照组),以发生排斥反应为无效,以患者术后就医所有花费为成本,应用药物经济学成本-效果法进行分析。结果观察组和对照组的成本分别为每月7 545元和每月7 843元,有效率分别为85.00%和86.67%,2组有效率比较差别无统计学意义(P>0.05);2组成本-效果比比较差别也无统计学意义(P>0.05);2组不良反应情况比较差别无统计学意义(P>0.05),但观察组血脂升高者明显高于对照组(P<0.05)。结论肾移植术后应用雷帕霉素或吗替麦考酚酯联合他克莫司、泼尼松的疗效和费用成本相当,可依据患者自身情况选择雷帕霉素或吗替麦考酚酯。

Cost-effectiveness analysis of rapamycin and mycophenolate mofetil in renal transplantation recipients

Objective To evaluate the cost-effectiveness of rapamycin or mycophenolate mofetil combined with tacrolimus,prednisone program for renal transplantation recipients.Methods Sixty cases were given rapamycine,tacrolimus and prednisone(observation group),and the other sixty cases were given mycophenolate mofetil,tacrolimus and prednisone(control group)after renal transplantation.Anyone who occured reject reaction considered to be invalid.All pay for disease as the cost,and then application pharmacoeconomic cost-effective method for analysis between two groups.Results The cost of observation group was 7 545 yuan per month,effective rate was 85.0%.The cost of control group was 7 843 yuan per month,effective rate was 86.67%.There were no significant difference of effective rates,side effects and ratio of cost-effectiveness between the two groups(P>0.05).But the hyperlipemia in observation group was more than control group(P<0.05).Conclusion The cost and effectiveness between two groups are similar.Rapamycine or mycophenolate mofetil should be selected depend on patients′ self-condition.
