江西省HIV - 1新发感染状况及空间扫描分析

摘    要:目的 分析江西省HIV - 1新发感染结果、影响因素和空间聚集性。方法 应用BED捕获酶联法对符合新发感染检测要求的新报告HIV - 1感染者的样本进行检测。结果 江西省2014年新报告HIV - 1感染者1 599名,检出新发感染者252名,新发感染者在新报告的HIV - 1感染者中占15.76%(252/1 599);不同设区市、年龄、婚姻状况、文化程度、感染途径和来源的样本的新发感染比例存在统计学差异;新发感染的发生与未婚(OR = 1.64,95%IC:1.10~2.45)和文盲(OR = 2.26,95%IC:1.28~3.99)等因素密切相关。通过回顾性空间扫描分析发现,2014年江西省HIV - 1新发感染存在2个聚集区(南昌市和景德镇市)。结论 与2011年相比,2014年的新发感染比例显著升高(P = 0.0192),加强对早期HIV - 1感染者的检测工作势在必行。同时,重点加强对2个新发感染聚集区的干预和疫情监管工作。

Abstract:Objective To analyze the results, influencing factors and spatial aggregation of recent HIV-1 infection in Jiangxi province. Methods All serum samples of newly reported HIV/AIDS cases in 2014 in Jiangxi province were tested using the BED-CEIA. Results A total of 1599 HIV/AIDS cases were newly reported in 2014 in Jiangxi province. Among the tested, 252 were regarded as recent HIV infections. The proportion of recent HIV infection among newly reported HIV/AIDS cases was 15.76%(252/1599), and there were significant differences between recent infections and long-term infections on the region, age, marriage, culture degree, propagation path and sources(P<0.05). A retrospective space scan revealed that there were two clusters of recent HIV-1 infection in Jiangxi province in 2014(Nanchang and Jingdezhen). Conclusion Compared with 2011, the proportion of recent infections increased significantly in 2014 (P=0.0192), so it is imperative to strengthen the detection of early HIV-1 infection. Meanwhile, emphasis will be placed on the intervention and supervision of the epidemic in the two clusters.
