急性脑梗死患者血糖水平与预后的临床观察

摘    要:目的:探讨急性脑梗死患者不同血糖水平与预后的关系。方法:85例患者住院24h内测空腹血糖,根据空腹血糖值分为高血糖组(血糖值≥6.1mmol/L)64例,其中,男34例,女30例;血糖正常组(血糖值〈6.1mmol/L)21例,其中,男11例,10例。比较两组不同血糖水平的疗效及临床表现与并发症情况。结果:高血糖组的存活率明显低于血糖正常组(P〈0.05),死亡率高于血糖正常组(P〈0.05),且高血糖组的临床表现,包括意识障碍、偏瘫、失语、延髓性麻痹、感觉障碍、病理反射的病例数均多于血糖正常组,其中偏瘫、失语、感觉障碍、病理反射的发生率与血糖正常组比较,差异有统计学意义(P〈0.05)。从并发症方面考虑,统计分析显示,高血糖组的应激性溃疡、肺部感染、心律失常、肾功能衰竭的发生率明显高于血糖正常组,经统计学分析显示,差异有统计学意义(P〈0.05)。结论:急性脑梗死患者发病后血糖越高,预后越差。因此,对急性脑梗死患者发病早期及时监测血糖水平,对判断病情、指导治疗及估计预后都具有一定的参考价值。

Blood glucose level in patients with acute cerebral infarction and prognosis of clinical observation

Abstract:Objective: To explore the different blood glucose levels in patients with acute cerebral infarction and prognosis. Methods: 85 patients were hospitalized 24 hours closed beta fasting glucose, fasting plasma glucose divided according to high glucose group (blood glucose≥6.1 mmol/L) 64 patients, male 34, female 30 cases; normal group (blood glucose6.1 mmol/L) 21 patients, male 11 cases, 10 cases. Different blood glucose levels were compared the efficacy and clinical manifestations and complications. Results: The survival rate of high blood glucose group was significantly lower than normal blood glucose group(P0.05), mortality rate higher than the normal group(P0.05), and high blood glucose groups manifestations, including disturbance of consciousness, hemiplegia, aphasia, medulla oblongata paralysis, sensory disturbances, the number of cases were pathological reflex than normal blood glucose group, but hemiplegia, aphasia, sensory disturbances, the occurrence rate of pathological reflex compared with normal blood glucose, the difference was statistically significant( P 0.05). Considerations from the complications, statistical analysis showed that high blood glucose group stress ulcer, pul - monary infection, cardiac arrhythmia, renal failure, failure was significantly higher than the normal group, after statistical analysis showed that the difference was statistically significance(P0.05). Conclusion: The incidence of blood glucose in patients with acute cerebral infarction higher the worse the prognosis. Therefore, the early onset of acute cerebral timely monitoring of glucose levels in judging the condition and guide treatment and prognosis have some value.
