经阴道超声引导下未成熟卵泡介入术治疗多囊卵巢综合征

目的评价经阴道超声引导下未成熟卵泡介入术（TUG-IFI）对多囊卵巢综合征（PCOS）的疗效。方法对接受辅助生殖技术治疗中低反应的PCOS患者行TUG-IFI，比较术前、后内分泌水平、排卵率、妊娠率以及相关超声参数的差异。结果87．50％患者在术后发生自发排卵，术后累积妊娠率为57．14％。术后血清睾酮（T）、黄体生成素（LH）及黄体生成素与卵泡刺激素的比值（LH／FSH）均显著低于术前（P〈0.01）。雌二醇（E2）、FSH和催乳素（PRL）的术前、后比较，差异均无显著性意义（P〉0．05）。HCG注射日，优势卵泡（〉18mm）和A型内膜明显增多，内膜明显增厚（P〈0．01），小卵泡数目较术前减少，卵巢体积缩小（P〈0．05）。结论TUC-IFI是治疗PCOS患者有效、创伤小的方法，能有效改善患者卵巢功能，有利于排卵和妊娠。

Transvaginal ultrasound-guided immature follicular intervention in patients with polycystic ovary syndrome

Objective To evaluate the therapeutic effect of transvaginal ultrasound-guided immature follicular intervention(TUG-IFI) in patients with polycystic ovary syndrome(PCOS).Methods Patients with PCOS undergoing poor performance in the treatment of assisted reproductive technology were collected in this research.Hormone profiles and ultrasound parameters were checked before and after TUG-IFI,while ovulatory and pregnancy rates and clinical outcome were assessed and compared.Results Ovulation occurred spontaneously in 87.50% of patients after TUG-IFI,and the accumulative pregnancy rate was 57.14%.After TUG-IFI,serum T,LH and LH-FSH ratio were significantly lower than preoperative(P<0.01),but differences were not observed in patients underwent TUG-IFI in E2,FSH and PRL concentrations(P>0.05).At the date of HCG injection,the amount of dominant follicle(>18 mm) and type A endometrial shape increased,endometrium thickened significantly(P<0.01),but the amount of small follicles and volume of ovaries decreased(P<0.05).Conclusion TUG-IFI is an effective,less invasive method in improving the ovarian function in the patients with PCOS.
