外伤性肝损伤的MSCT诊断价值分析

日的探讨MSCT扫描检查在肝脏创伤中的诊断价值。方法选择我院诊治的106例肝脏创伤患者,全部患者在伤后2Omin至3d内进行了64排螺旋CT检查,其中62例患者行增强扫描,并按美国创伤外科学会（AAST）及我国王正国院士分级方法进行分级。结果肝脏创伤患者I级24例,II级25例,III级34例,IV级14例,V级7例。其中,肝包膜下血肿56例,肝挫伤61例,单发或多发肝脏撕裂伤45例,肝实质内血肿30例,肝破裂21例,肝门汇管区损伤8例。结论MSCT扫描检查能明确肝创伤的范围、程度,对肝脏创伤的诊断和治疗方案的选择具有重要指导意义。

Analysis of Diagnostic Value by MSCT Scanning in Hepatic Trauma

Objective To evaluate the value of MSCT in hepatic trama. Methods 106 patients with hepatic trauma were selceted as the object in the study,all of the patient s under- went the MSCT examination in 20min ~ 3days after the injury, and 62 patients received contrastenhanced MSCT at the same time.We classified severity of hepatic injnry by AAST （American Association for the Surgery of Trauma） and Academician WANG grade system. Results Hepatic injury was graded using the MSCT scan classification：grade I 24 cases,grade lI 25 cases,gn~de III 34 cases,grade IV 14 cases and ~~rade V 7 cases.There were hepatic subcapsular hematoma 56 cases,parenchymal contusions 61 cases,laceration 45 cases,paren- ch＇~mat hematoma 30 cases,hepatic rupture 21 cases,hepatic periportal injury 3 cases. Con- clusion MSCT can identify the extent and degree of the hapatic trauma with the affluent information of the injury,and it plays a vitalrole in the diagnosis of the hepatic injury and the choose of the treatment.
