糖尿病对急性心肌梗死患者心室功能的影响

摘    要:目的探讨糖尿病对急性心肌梗死(AMI)患者心功能的影响。方法AMI患者126例,其中男101例,女25例;平均年龄(59±11)岁;根据有无糖尿病分为糖尿病组(36例)和非糖尿病组(90例)。所有患者AMI发病后2~7d应用平衡法核素心血池显像判定左心室功能及心室收缩同步性。结果2组间患者的年龄、性别比、高血压病史及吸烟、饮酒史间差异无统计学意义(P〉0.05)。糖尿病患者血脂较高,但未达到统计学差异。伴有糖尿病的AMI患者心功能不全的发生率明显增加,心脏收缩及舒张功能均显著受损,而其心室收缩同步性参数比无糖尿病患者显著减低。结论糖尿病不仅加重急性心肌梗死患者心室的收缩及舒张功能障碍,并且使整个心室心肌功能弥漫性减低。

Abstract:Objective To investigate the influernce of diabetes on left ventricular function of patients with acute myocardial infarction. Methods The present study was performed in 126 patients [ ( 101 men,25 women, age range : (59±11.48 ) yr) ] who suffered from primary acute myocardial infarction. Except for the study of the history of obsolete myocardial infarction, severe valvular heart disease, dilated and hypertrophic cardiomyopathy, all patients underwent equilibrium radionaclide angiography(ERNA) between 2 and 7 days after the onset of AMI to evaluate the left heart function and ventricular systolic synchrony ( VSS ). Results There was no significant difference in the age ,sex ,hypertension, addiction for smoking and drinking between the two groups. The injury of heart function including the systolic and diastolic function of patients with diabetes was found, as compared with that of patients without diabetes. LVEDP of patients with diabetes was higher than that of patients without diabetes. The patients with diabetes had better VSS, as compared with patients without diabetes, and PSD, PS and FWHM of patients with diabetes were redused by 50%, as compared with patients without diabetes( P 〈 0.05 ). Conclusion Diabetes can aggravate the left ventricular function in patients with acute myocardial infarction. Furthermore, it can reduce all myocardium function in the whole ventricle.
