介入治疗在妇科恶性肿瘤中的临床应用

目的:研究介入治疗在妇科恶性肿瘤中的临床应用价值,并对其并发症进行探讨。方法:对37例经病理证实的妇科恶性肿瘤共作了78次介入治疗,每人1～3次。全部病例均采用Seldinger穿刺术,经一侧股动脉插管(一般为右侧股动脉),行一侧或两侧盆腔动脉内灌注化疗药物和(或)栓塞的方法。结果:所有病例DSA造影均表现为相应区域内的肿瘤染色和供血血管增粗。8例获得了根治性手术切除,术中出血明显减少,总有效率达82%。本组病例介入治疗后有1例(占2.7%)出现了臀部、外阴部皮肤组织缺血坏死较为严重的并发症。结论:介入治疗中晚期妇科恶性肿瘤是一种安全而有效的姑息性疗法。

The clinical application of interventional therapy in gynecologic malignancy

Objectiv e :To study the clinical application of interventional therapy in the treatment of gynecologic malignan-cy,and to discuss the complications associated.Met hods:Seventy-eight procedures of interventional therapy were performed in3 7patients with pathologically confirmed gynecologic malignancy,with one to thr ee procedures per case.The Seldinger tech-nique was used in all patients.The catheter was introduced via femoral artery on one side(mostly on the right sid e),and the combined antineoplastic agents were infused into contra-lateral in ternal iliac artery and(or)ipsilateral branches supplying the in-volved area .Common ilinc arteries and inferior mesenteric arteries were also used in some cases.Results:The hemorrhage was obviously reduced on operation in8patients after interventional therapy.The total effective rate was82%.One patient (2.7%)developed severe skin and subcutaneous necrosis(erosion or ulcerat ion)on the buttock and valvae.Conclusion:The interven-tion therapy in advanc ed gynecologic malignancy is a safe and effective palliative treatment.
