早期颈内静脉置管在抢救上消化道大出血患者中的效果分析

目的探讨早期颈内静脉置管和传统静脉输液在上消化道大出血的抢救过程中的效果。方法将90例上消化道大出血患者随机分为实验组和对照组各45人,实验组使用颈内静脉置管在监测中心静脉压下快速输液,对照组进行传统静脉输液,在30min,1h,2h分别观察记录所有患者的收缩压、脉搏和尿量,比较两组的总有效率、显效率、有效率和无效率。结果实验组和对照组总有效率分别为97.8%和86.6%,无效率分别为2.2%和4.4%,有显著性差异(P<0.05),实验组和对照组显效率分别为84.4%和45.6%,有效率分别为83.3%和40.2%,差异有统计学意义(P<0.05)。结论早期颈内静脉置管是一种快速有效的抢救手段。

The Effect Analysis of Internal Jugular Vein Catheterization in Treatment of Upper Gastrointestinal Hemorrhage

Objective Study of internal jugular vein intubation and forearm venous transfusion in hemorrhage of upper digestive tract in the process of rescuing.Methods 90 cases of upper digestive tract hemorrhage were randomly divided into experimental group and control group each 45 person,the experimental group using internal jugular vein catheter infusion,the control group use traditional intravenous transfusion,and compared a success rate of catheterization.Observation records of all patients with SBP,CVP,pulse,urine volume,PCV,compared with two groups of the total eff iciency 、signifi cant effi ciency 、effective and ineff iciency.Results The experimental group’s and the control group’s total effective rate were 97.8% and 95.6%,ineffi ciency were 2.2% and 4.4% with no signifi cant difference(P>0.05),the experimental group’s and the control group’s signifi cant effi ciency and effective were 84.4% and 55.6%、13.3% and 40.3%,the difference was statistically signif icant(P<0.05).Conclusion Internal jugular vein catheter salvage massive upper gastrointestinal hemorrhage is a safe,effective rescue means.
