益肾康颗粒防治慢性肾盂肾炎的临床免疫学研究

目的:探讨慢性肾盂肾炎(CPN)与免疫功能的关系以及中成药益肾康颗粒对CPN的作用机制。方法:将120例CPN患者随机分为益肾康颗粒组、三金片组、西药组,另设健康组。采用单克隆抗体、免疫比浊、放免分析及放免平衡法对CPN患者治疗前、后及健康组的外周血T淋巴细胞亚群、血清免疫球蛋白、补体、尿液分泌型IgA以及白细胞介素-2(IL-2)水平进行测定。结果:CPN患者与健康人群比较,血清免疫球蛋白、补体、尿液分泌型IgA含量均显著降低,IL-2水平显著增高(P<0.05或P<0.01)。治疗后益肾康颗粒组的各项指标恢复正常,与健康组无显著差异;而三金片组和西药组虽有所恢复,但与健康组比较仍具显著性差异(P<0.05或P<0.01)。结论:CPN患者在慢性阶段存在着一定程度的细胞免疫、体液免疫功能紊乱。益肾康颗粒能纠正免疫系统功能紊乱的状态,有效地控制CPN的复发。

Prevention and Treatment of Chronic Pyelonephritis with Yishenkang Granule:Clinical Immunology Study

OBJECTIVE： To study the links between chronic pyelonephritis （CPN） and immune function, and the action mechanism of Yishenkang granule on CPN. METHODS： A total of 120 CPN cases were randomly assigned to Yishenkang granule group, Sanjing tablet group and western medicine group, which were compared with healthy control group. The levels of the peripheral blood T lymphocyte subsets, serum immunoglobulin, complement, secretory IgA in urine and the cell factor IL- 2 in CPN cases before and after treatment were detected by monoelonal antibody method, agar diffusion method, radioimmunoassay, radioimmune balancing method and compared with those in healthy control group. RESULTS： In CPN cases compared with healthy control group, the levels of serum immunoglobulin, complement, secretory IgA in urine were signifi- cantly lower while the level of cell factor IL- 2 were signifienatly higher （P 〈 0.05 or P 〈 0.01） .All of the indexes returned to normal levels and nonsignificant as compared with healthy control group after Yishenkang granule intervention; however, the indexes improved to a certain degree after intervention with Sanjing tablet or western medicine, but which were significantly different from health control group（P 〈 0.05 or P 〈 0.01） .CONCLUSION：There were cellular immunity dysfunction and humoral immune dysfunction in CPN patients in chronic stage, but which can be corrected and effectively controlled by Yishenkang granule.
