带血管蒂尺神经皮下前置术治疗肘管综合征57例临床分析

目的:评价带血管蒂尺神经皮下前置术治疗肘管综合征的疗效及其影响因素.方法:对1995～2006年采用带血管蒂尺神经皮下前置术治疗的57例肘管综合征患者的疗效进行评价,同时分析年龄、病程、病因、术中发现对手术预后的影响,观察感觉障碍、爪形指、第1背侧骨间肌萎缩、手指内收外展受限等的恢复进程,并检查肘部切口瘢痕触痛情况.结果:肘管综合征轻度27例疗效优良率为96.3%,中度15例疗效优良率为93.3%,重度11例疗效优良率为9.1%,4例肘部手术瘢痕有叩击痛.结论:带血管蒂尺神经皮下前置术是治疗肘管综合征可行、有效的方法.年龄小、病程短、症状以感觉障碍为主者术后恢复时间短、疗效好.

Clinical analysis for 57 cubital tunnel syndrome using anterior subcutaneous transposition of the vascularized ulnar nerve

Objective: To evaluate the treatment results for cubital tunnel syndrome with anterior subcutaneous transposition of the vascularized ulnar nerve and its related factor.Methods: From 1995 to 2006, we had treated 57 cases of cubital tunnel syndrome with anterior subcutaneous transposition.Meantime the influence of age,course,etiology,operative findings on prognosis was addressed.The recovery processes,including sensory dysfunction,claw fingers,the atrophy of the first dorsalinterosseous muscle,the deformity of finger abduction were analyzed.The scar tenderness at the elbow was also examinded.Result: There were 27 mild cases with 96.3% satisfactory rate,15 moderate cases with 93.3%satisfactory rate and 11 severe cases showed only 9.1% satisfactory results,4 cases had percussion tingling sensation.Conclution: The anterior subcutaneous transposition of the vascularized ulnar nerve is a reliable and effective surgical option in the management of cubital tunnel syndrome.the following factors,including relative younger patients,shorter course,mainly sensory dysfunction were related to the shorter recovery period and better treatment results.
