海洛因依赖患者局部脑血流灌注改变研究

摘    要:目的:探讨海洛因依赖者局部脑血流(rCBF)表现,并提高对本病的认识。方法:141例海洛因依赖者静脉注射99mTc-ECD 740MBq,30min后行局部脑血流灌注SPECT显像。结果:141例海洛因依赖者136例(96.5%)见rCBF减低。46例(33%)显示1处rCBF减低,68例(48%)为同时存在2处rCBF减低,17例(12%)为同时存在3处rCBF减低,3例(2.1%)为同时存在4处rCBF减低,2例(1.4%)为同时存在5处rCBF减低。海洛因依赖者rCBF减低主要见于顶叶、额叶、枕叶和颞叶。结论:海洛因依赖者可能存在顶叶、额叶和颞叶功能低下,rCBF检测对海洛因依赖有诊断价值。

Abstract:Objective: To investigate the changes of regional cerebral blood flow(rCBF) of heroin addicts and to improve the knowledge of this disease. Methods: One hundred and forty-one heroin-dependent individuals were scanned with single photon emission computed tomography(SPECT) and their rCBF were obtained 30min later after injection of 99mTc-ethylene cysteinate dimer(ECD) 740MBq. Results: A decrease in rCBF was found in 136 patients(96.5%). Forty-six(33%) patients had one low rCBF focus. Sixty-eight(48%) patients had two low rCBF foci. Seventeen(12%) patients had three low rCBF foci. Three(2.1%) patients had four low rCBF foci. Two(1.4%) patients had five low rCBF foci. The decrease of rCBF was distributed mainly in parietal lobe- temporal lobe- frontal lobe. Conclusions: Low rCBF of parietal lobe- temporal lobe- frontal lobe may be detected in heroin-addicted patients. These results suggest that rCBF is of value in the diagnosis of heroin addicts with low rCBF of parietal lobe- temporal lobe- frontal lobe and thalamencephalon.
