检测血清血管内皮生长因子与血红蛋白在肿瘤患者中的意义

摘    要:目的 探讨血清血管内皮生长因子 (VEGF)在肿瘤患者诊治中的临床意义以及血红蛋白 (Hb)浓度与VEGF水平间的关系。方法 采用酶联免疫方法检测未经任何治疗的肿瘤患者血清VEGF浓度 ,同时常规检测患者Hb含量。结果 胃癌、肺癌、结直肠癌等肿瘤患者血清VEGF较正常对照差异有显著意义 (P <0 .0 1)。相关分析表明 ,肿瘤患者Hb含量与血清VEGF浓度呈负相关 (r =- 0 .2 89,P <0 .0 1)。低Hb(<12 3.5g/L)水平组肿瘤患者血清VEGF含量 [(6 95 .3± 5 72 .4 )ng/L]与高Hb(>12 3.5g/L)组 [(42 3.7± 35 7.5 )ng/L]差异有显著意义 (P <0 .0 5 )。 2 8例手术前Hb浓度正常 (>110g/L)的患者 ,17例手术后 1周Hb低于正常者 ,其中 13例术后血清VEGF升高 ,11例手术后 1周Hb仍正常者中只有 3例血清VEGF浓度较术前升高 ,两组间差异有显著意义 (P >0 .0 5 )。结论 血清VEGE有可能作为肿瘤辅助诊断的指标 ,低的Hb浓度很可能是患者血清VEGF升高的诱因 ,提示及时纠正肿瘤患者低Hb含量有可能是预防血清VEGF水平升高的有效途径之一。

Abstract:Objective To observe the relationship between hemoglobin (Hb) and vascular endothelial growth factor (VEGF) in serum of the patients with cancer. Methods Enzyme linked immunosorbent assay (ELISA) was used to detect VEGF levels in serum of the patients with cancers. Results The serum level of VEGF in the cancer group are significantly higher than that in the control group. The serum level of VEGF in patients with low level of hemoglobin (<123.5 g/L) are higher than that in those with high level of hemoglobin (> 123.5 g/L) ( P <0.05). There are negative correlation between the Hb and the serum VEGF in the patients with tumor( r =-0.289, P <0.01). In the 28 cases, 17 of them had low level of Hb after the operation in which 13 patients had VEGF increased; 11 of them had high level of Hb where only 3 patients had VEGF increased( P <0.05). Conclusion The serum VEGF may be an assisting diagnostic marker for cancers. The low hemoglobin level in serum may stimulate the secret of VEGF by hypoxia inducing. The result implicate that keeping the high Hb levels may block the increase of VEGF in serum.
