350例冠心病患者24h动态心电图分析

目的 对350例冠心病患者24h动态心电图监测的结果进行分析探讨，从而评价12导联动态心电图对冠心病的诊断价值。方法 使用ECG／ABHOLTER12TOP版检测仪，通过24h心电监测，观察冠心病患者24h动态心电图检出的心律失常、心肌缺血、心率变异性（HRV）的有关资料。结果 检出心肌缺血248例，其中无症状心肌缺血（SMI）189例．141例不伴有心绞痛发作。检出心律失常661例次，冠心病患者常见的心律失常依次为室上性早搏、室性早搏、房性心动过速。350例患者中检出102例HRV降低（29％），其中无症状心肌缺血69例（67％）。结论 无症状心肌缺血可作为不稳定型心绞痛发作的一项重要预后指标，冠心病人特别是SMI与HRV有一定关系。

DCGs for 24 h of 350 patients with coronary heart disease

Objective To analyze and approch the monitored results of the DCGs for 24 h of 350 patients with coronary heart disease and evaluate the diagnostic value of 12 lead DCG to coronary heart disease. Methods With the detector of ECG/ABHOLTER 12 TOP edition,the patients with coronary heart disease have been monitored by means of cardiograms for 24 h. Then the data, that are detected by the DCGs, relative to arrhythmia, clinical or subclinical myocardial ischemia and variabilities of heart rate have been inspected. Results 248 patients of myocardial ischemia had been detected, among them,189 patients were of subclinical and 141 patients had not an attack of angina pectoris. 661 patients and times of arrhythmia had been detected. Common kinds of arrhythmia of the patients with coronary heart disease is order of supraventriclar extrasystole,ventricular extrasystole and atrial tachycardia. In the 350 patients HRV reductions of 102 patients(29%) had been detected,among them, 69 patients(67%) were of subclinical myocardial ischemia. Conclusion It is pointed out that subclinical myocardial ischemia can be made as an important prognostic index having an attack of angina pectoris of unstable type. Especialy,in the patients with coronary heart disease SMI have a certain relationship with HRV.
