透明质酸钠与几丁糖预防甲状腺术后粘连效果比较分析

目的：探讨透明质酸钠和几丁糖分别用于甲状腺术后粘连的治疗效果。方法：甲状腺手术患者84例根据入院时间分为甲、乙两组各42例，为防止术后粘连，甲组采用透明质酸钠3～6ml均匀涂抹于创面、颈前肌群深面和颈阔肌深面，乙组采用3～6ml均匀涂抹于上述位置。术后5～7d拆线出院，3个月后评估预防粘连效果，比较两组临床疗效及不良反应发生情况。结果：甲组粘连率为14．3％（6／42），略高于乙组7．1％（3／42），但差异无统计学意义（P〉0．05）；两组患者均无渗血和过敏等不良反应，仅甲组出现2例（4．8％）轻微感染者，两组不良反应发生率差异无统计学意义（P〉0．05）。结论：透明质酸钠和几丁糖均有预防甲状腺术后粘连的效果，两者预防有效率和不良反应基本相当。

Effect Comparative Analysis of Sodium Hyaluronate and Chitosan in the Prevention of Thyroid Postoperative Adhesion

Objective： To investigate and compare the effect of sodium hyaluronate and chitosan in the prevention of thyroid post-operative adhesion. Methods： Totally 84 patients with thyroid operation in our hospital from August 2012 to August 2013 were select- ed, and according to the admission time, the patients were respectively divided into group A and B with 42 cases each. In order to prevent postoperative adhesion, group A was treated with sodium hyaluronate, and group B was treated with chitosan. The clinical curative effect of the two groups was compared. Results： The adhesion rate in group A was 14.3% （6/42）, which was higher than that in group B （ 3/42 7.1% ）, while the difference was not statistically significant （ P 〉 0.05 ）. All the patients showed no bleeding, allergies and other adverse reactions, only in group A, 2 mild infection （4.8%） occurred. The adverse reactions in the two groups were without significant difference （ P 〉 0.05 ）. Conclusion ： Sodium hyaluronate and chitosan both have the preventive effect on thyroid postoperative adhesion with similar efficiency and adverse reactions.
